Position Summary
This position may be eligible for a hybrid work arrangement that may include a partially remote work location, consistent with System Office policy. UNC Chapel Hill employees are generally required to reside within a reasonable commuting distance of their assigned duty station.

This position will support the CATCH-Johnston program. The Patient Care Assistant is responsible for supporting care delivery as part of the interdisciplinary care team through serving as the in-person contact at the community virtual care access sites, patient resource/referral coordination, and completing initial calls to caregivers seeking care for children and adolescents in their care. Applicants may also be asked to complete tasks assigned by the Program Director and the Medical Director to support CATCH-Johnston. Such tasks include, but are not limited to, connecting with stakeholders, community education, direct patient outreach, quality metric reviews, and preventative gap closure. The Patient Care Assistant will be trained to use UNC’s electronic medical record. This is a hybrid position, with some days/times where the Patient Care Assistant will be able to work from home each week.

Minimum Education and Experience Requirements
Associate’s degree in a related Human Services discipline and two years of experience related to the area of assignment, or equivalent combination of training and experience. All degrees must be received from appropriately accredited institutions.
Required Qualifications, Competencies, and Experience
Working knowledge of behavioral health conditions.
Preferred Qualifications, Competencies, and Experience
* Work experience with the population served (children and/or adolescents and/or their families).
* Work and/or lived experience navigating the behavioral system
* Experience with outreach or community-based service models and knowledge of local community resources and service systems in program catchment area.
* Bilingual English/Spanish highly desired
* Community outreach experience in the geographic area of the program, [Johnston County], highly desired, but not required
* Telephone skills with excellent customer service techniques.
* Strong verbal, written, and interpersonal skills.
* Applicants with relevant experience, but no official post high school/GED degree or certifications, will be considered.
Required Licenses/Certifications Special Physical/Mental Requirements
Must have reliable transportation to and from virtual care access sites on the days the community access sites are opened (other days the Patient Care Assistant will be able to work from home).
