This position may be eligible for a hybrid work arrangement that may include a partially remote work location, consistent with System Office policy. UNC Chapel Hill employees are generally required to reside within a reasonable commuting distance of their assigned duty station.
This is recruitment is for two vacancies (positions 20048585 and 20048587). By applying you will be considered for both opportunities.
This position will support the CATCH-Johnston program. The Patient Care Assistant is responsible for supporting care delivery as part of the interdisciplinary care team through serving as the in-person contact at the community virtual care access sites, patient resource/referral coordination, and completing initial calls to caregivers seeking care for children and adolescents in their care. Applicants may also be asked to complete tasks assigned by the Program Director and the Medical Director to support CATCH-Johnston. Such tasks include, but are not limited to, connecting with stakeholders, community education, direct patient outreach, quality metric reviews, and preventative gap closure. The Patient Care Assistant will be trained to use UNC’s electronic medical record. This is a hybrid position, with some days/times where the
Patient Care Assistant will be able to work from home each week.
* Work and/or lived experience navigating the behavioral system
* Experience with outreach or community-based service models and knowledge of local community resources and service systems in program catchment area.
* Bilingual English/Spanish highly desired.
* Community outreach experience in the geographic area of the program, [Johnston County], highly desired, but not required.
* Telephone skills with excellent customer service techniques.
* Strong verbal, written, and interpersonal skills.
